Now hiring qualified Nurse Practitioners with Home Health experience across West Virginia!

This is a hybrid position - both in person and remote is required.

Nurse Practitioner Pay:

• $4,200 weekly gross working 40 hours.
• $105/hour.
• $157.50/hour overtime.
• Plus travel bonus to assignment if eligible.
• Local and pay packages available.
• Weekly Direct Deposit every Friday.
• W2 position with benefits Available; Medical, Dental, Vision insurance, 401K.
• Hotel, flight provided.
• Breakfast, lunch provided.
• $50/day for other meals/snacks.

Schedule and Timeline:

• Candidates must be OK with driving 60 miles around the metro area (Mileage is paid at $0.44/mi). Mainly going to be remote – must live within 60 miles of the area you are submitted to – must be okay with patient visits 3/days week.
• Full 40-hour work week.
• 2-4 patients/day.
• 26 week contract – option to go perm!
• Clock runs from door to door – looking for flexibility as well – OT is welcomed.
• Chronic kidney disease, nephrology care.
• $70/monthly reimbursement for cell phone bill.
• In-person training in Nashville for two days.

Nurse Practitioner Job Details:

• Conducts assessments, which include comprehensive annual wellness exams on patients both in the patients' home and in the virtual environment.
• Counsels and educates patients and families about benefits and programs available to help them live healthier lives.
• Documents items such as: appropriate chief complaint, all applicable diagnoses, past medical, family, and social history, review of systems, examinations, medications, allergies, assessment, and plan.
• Responsible for the coordination of care with primary care providers, specialists, and appropriate ancillary services.
• Completes all documentation and paperwork in a timely manner.
• Maintains quality of care standards as defined by the practice.
• This position will not be office-based but will be remote in state in which employed and will need to attend periodic training/meetings outside of that state.
• Deliver evidence-based, timely care in a manner that reduces avoidable hospitalizations, maximizes quality of life, and puts patient health and satisfaction first.
• Prescribe medications, order tests, and collaborate with patient’s Monogram physician.
• Perform effectively, as reflected by improved patient quality outcomes, which will be measured and reported daily.
• Facilitates closing gaps in care by educating patients about preventive monitoring and working with physician practices to schedule diagnostic testing.
• Assists patients with enrolling to access educational videos.
• Participates in the integrated care team meetings.
• Knowledge of disease diagnosis and prevention.
• Make assessment of patient's health status.
• Develop treatment plan.
• Implement a plan consistent with appropriate plan of care.
• Follow-up and evaluate patient's status.
• Other duties as assigned.

Home Health RN / Registered Nurse Qualifications:

• West Virginia NP License in good standing.
• Active AHA BLS certification.
• Clean driving record.
• Required to travel up to 60 miles within territory.
• Must have a dependable vehicle, valid drivers license, and current auto insurance in accordance with state laws.
• Able and willing to travel within branch/office coverage area.
• Bilingual (Spanish/English) preferred.
