We support patients in their recovery by delivering tailored care in the comfort of their homes. We are seeking a licensed clinician with a heart for patient advocacy and a passion for building professional relationships to join our growing team. Position Overview As a Licensed Clinical Discharge Liaison, you will be the vital link between hospitals/skilled nursing facilities and our home care services. Your clinical expertise will help guide appropriate patient referrals, while your relationship-building skills will strengthen our partnerships with referring providers. This is a unique blend of patient advocacy, clinical evaluation, and healthcare sales. Key Responsibilities

  • Collaborate with hospital and SNF discharge planners, case managers, and physicians to identify patients suitable for home care.
  • Conduct clinical assessments to determine eligibility and service needs.
  • Provide education to patients and families about our home care services prior to discharge.
  • Serve as a trusted point of contact for referral sources.
  • Coordinate with internal care teams to ensure a seamless intake process.
  • Maintain a consistent presence at assigned hospitals/SNFs to build trust and drive referrals.
  • Track referral outcomes and meet monthly admission targets.
Qualifications
  • License as an RN, LPN, or LCSW (preferred)
  • Minimum 2 years of clinical experience in a hospital, SNF, or home health setting
  • Experience in discharge planning or case management strongly preferred
  • Strong interpersonal and communication skills
  • Highly organized, self-motivated, and goal-oriented
  • Familiarity with Medicare/Medicaid and payer sources a plus
  • Valid driver’s license and reliable transportation
Compensation
  • Total Compensation: $60,000 – $110,000 (depending on license and experience)
  • Bonus Structure: Performance-based bonuses tied to referrals and admissions
