The Work You Will Do

This includes, but is not limited to, assisting the community adult client to maximize social, emotional and cognitive functioning that promote optimum independence and improve quality of life. Communicate with clients, families/caregivers, service providers and other professionals to collaborate and coordinate services. Share our desire to help clients live at home and be fully independent. The successful candidate will work directly with members of both the English and French communities.

Education and Experience

• ideal candidate would have experience in Home Health, mental health and psychosocial assessments
• Bachelor’s Degree (Required)
• registered member of the Ordre des travailleurs sociaux et des thérapeutes conjugaux et familiaux du Québec

similar duties were completed

• a valid driver’s license and reliable vehicle for commuting
• A clear police criminal record and able to get a Vulnerable Sector Screening if required
