Job Summary (List Format):


- Conduct comprehensive Physical Therapy assessments for referred patients, considering physiological, psychosocial, environmental factors, and health history.
- Integrate assessment data to develop and implement individualized plans of care that promote patient function and safety.
- Maintain annual HHRG (Home Health Resource Group) values at or above national standards and use these values to guide visit frequency and interdisciplinary utilization.
- Achieve annual patient outcomes that meet or exceed national benchmarks.
- Refer patients to appropriate community resources as needed.
- Evaluate clinical needs, initiate and revise patient care plans, and document all problems and interventions accordingly.
- Participate in interdisciplinary patient case conferences to evaluate and adjust care plans.
- Supervise and instruct Home Health Aides, ensuring documented oversight every 14 days.
- Notify physicians and other care providers promptly about significant changes in patient conditions.
- Complete all admission, clinical, and discharge paperwork accurately and on time.
- Ensure clear, concise documentation reflecting skilled care, teaching, direct care, and patient/caregiver responses.
- Obtain and clarify valid physician orders for all care provided.
- Plan and coordinate patient discharges, communicate with patients and physicians, and complete all discharge documentation.
- Consistently demonstrate service excellence in alignment with the organization's CARES values: compassion, accountability, respect, excellence, and service.
