This position is located in Tulsa, Oklahoma.

Health Care Management Nurse

Annual Salary:

Level I: - Y15A -  $58,055.13 +

Level II: - Y15B - $62,409.27 +

Travel is occasional  - Must possess a valid driver's license and must maintain required car insurance.


Minimum Qualifications:

Level I:

  • Possession of a valid permanent Oklahoma license as a registered nurse as approved by the Oklahoma Board of Nursing or Registered Nurse Licensure Compact (eNLC)
  • Two years of professional nursing experience

Level II:

  • Possession of a valid permanent Oklahoma license as a registered nurse as approved by the Oklahoma Board of Nursing or Registered Nurse Licensure Compact (eNLC)
  • Three years of professional nursing experience

Job Responsibilities

  • Attend Interdisciplinary Team meetings of assigned service recipients to assist in identifying and providing recommendations regarding health concerns and long-range planning needs.
    • Educates team regarding health care needs.
    • Complete and update a health acuity tool as needed to assist with the identification of health needs of the service recipient and training requirements for staff who provide direct care to the service recipient.
    • Review and assist with maintaining health history in service recipient’s electronic record.
    • Develop a Nursing Service Support Plan (NSSP) for service recipients who require nursing services.
    • Seek out existing generic community health related services to meet the service recipient’s health needs and actively develop (locate, contact and identify) services when none are available.
    • Complete assigned assessments and submits reports to include emergency Preadmission Screening and Resident Review (PASRR) assessments, routine and reconsideration PASRR assessments, 24 Hour Mortality Report upon notification of service recipient’s death, and service recipient transitions and transfers.

KNOWLEDGE, SKILLS, AND ABILITIES (KSA's)

  • KSAs required include: the knowledge, skills, and ability to analyze complex health concerns and issues, including the evaluation of adherence to established health care standards and the effectiveness of health management plans and/or programs and recommending changes or revisions.
  • Employees have a high degree of independence in planning and completing work assignments and may function as the lead person on some special projects. Also requires professional nursing knowledge in reviewing, auditing, and analyzing nursing and healthcare services provided by direct care providers to insure that the appropriate healthcare resources are used at the appropriate level to meet the health care needs of clients.
